Create your own advance directives and living will. The Incendant Advance Directives application uses video and a guided interview format to explain Advance Directives and Living Wills. The application then walks you through your choices helping you complete your own Advance Directives. Upon completion of the guided interview you will receive via email a completed document of the Advance Directives Choices that can be signed and distributed to family, care givers and others as needed.

Advance directives are your written wishes regarding certain healthcare. The purpose of putting your wishes on paper is so that if at some future time you can’t speak; healthcare providers will still know what you want. This application allows you to:

1) Designate a HEALTH CARE AGENT
2) Create a LIVING WILL listing your choices about medical care if you have a terminal condition
3) Specify Do Not Resuscitate (DNR) orders
4) Document these choices and create a signed copy with two witnesses.

After watching the video segments and entering your choices a completed document that includes your choices will be sent to your email. You will need to print that document and have it signed by two witnesses for your choices to be effective. In some states you may also need to have your document notarized.
