1910年 (明治43年) 12月1日、東雲堂書店より初版が刊行された。啄木は同年に朝日新聞社校正係として『二葉亭全集』校正を行っており、9月に朝日歌壇が設けられると選者を務めている。同年6月に大逆事件が起こると社会主義評論も手がけ、10月には長男・真一が誕生しているが、まもなく死去している。
序文を書いている「藪野椋十」とは、当時の啄木の勤務先である東京朝日新聞で社会部長を務めていた上司にあたる渋川玄耳（柳次郎）のことである。献辞では啄木を経済的・精神的に支えた宮崎郁雨・金田一京助への感謝や、夭折した長男・真一への哀悼が語られている。
挿絵は画家の名取春仙が描いている。
「我を愛する歌」「煙」「秋風のこころよさに」「忘れがたき人人」「手套を脱ぐ時」の五部構成。551首が入収。歌人・国文学者の土岐哀果（善麿）は同年4月に歌集『NAKIWARAI』（ローマ字ひろめ会）を刊行し、ヘボン式ローマ字三行表記の短歌を提唱した。なお、土岐善麿と啄木は1911年1月に知り合い、雑誌『樹木と果実』の発刊を計画するが、これは実現していない。善麿は病床の啄木を生活面においても支え、没後には啄木作品の刊行にも携わっている。
啄木は『一握の砂』においてこれに倣い、三行分けによる散文的なスタイルの短歌は、若い世代を中心に多くの追従者を生んだ。啄木の北海道時代の回想や、故郷・岩手への望郷を歌った歌、貧困と挫折で鬱屈した心情などを歌った数が収められている。特に啄木の郷里の岩手県では、刊行前後から地元紙に啄木の作品が掲載されたこともあり、その影響は大きかった。その一人が旧制盛岡中学校の後輩で当時在学中だった宮沢賢治で、本作の刊行と同時期に短歌創作を始めており、啄木の影響と推察されている。
歌風は、徹底的な生活派であり、雄大な情景よりは、ごくありふれた人間的な感覚を歌ったものが多い。もっとも過激な例でいえば「どんよりと くもれる空を 見ていしに 人を殺したく なりにけるかな」といった短歌さえ所収されているほどである。
