What is mForm ISMS?

The mobile Form ISMS App is a cutting edge application that enables you to complete and submit forms,
that have been sent to you from ICT MBPJ.

How does mForm ISMS work?

If you have received a request to download the mForm ISMS App from an organisation this means they would like to you complete their forms via the app,
which saves time, paper and makes the whole process much easier. Once you have set up your IC number and have added your name and email you
will then be able to receive, save and submit forms.

Note: This application is for employers and employees MBPJ only.

Jika anda menerima satu mesej emel untuk memuat turun aplikasi mform ISMS ini bermakna anda diminta mengisi borang dengan lengkap melalui aplikasi dan seterusnya
dapat menjimatkan masa , kertas dan menjadikan keseluruhan proses lebih mudah. Sebaik sahaja anda telah mempunyai nombor kad pengenalan,nama dan emel
MBPJ maka anda akan dapat menghantar borang permohonan ini dengan mudah.
